Close. I'm looking for the instructions on the form Authentication, to choose whether or not the form requires the user to authenticate first.

That would be based on whether or not the service is configured as publicly requestable. If it is not, then that service would always require authentication before you could access the form page for the service.
To check whether a service is publicly requestable you have to view its Form tab by editing the service in the client portal. There is a setting on the Edit interface's Form tab for whether the service should be publicly requestable or not. - Mark Sayers Wed 11/2/22 9:37 AM

Is there a KB article on the information that needs to go into the SAML fields? including the Required Claim Name.
