Re-use Same Attribute on Form

Hello,

I am building a ticket request form that would need to re-use some of the same attributes more than once on the same form. One attribute is a dropdown with a list of account codes and the other is a textbox to collect a dollar amount. The use case is for a budget rep to submit a budget transfer request, where a certain dollar amount will be transferred from one account to another (hence the need to collect 2 account codes). A submitted budget transfer request may also include multiple fund transfers, which would necessitate a way to duplicate the attributes as needed. When I first add the attribute to the form in Design mode, it no longer shows it on the list of available attributes. So, it looks like I can only use it one time. Is there a way to reuse the same attribute or do I need to create multiple variations of the same type?
