Images in KB not displaying

We've had a reoccurring issue in which some KB articles are not displaying images, but the space for the image is there. This has been reoslved by editing the KB article, downloading the current image from the article and uploaded again.

This has been tested on multiple, up-to-date browsers and happens on them all. I've attached a screenshot

 

We are on-prem, running 11.3. Is there any reason for this, and is there any action that we can take to resolve this permanently?

Hello Eugene,

I can only confirm that this does not happen in the live SaaS environment currently on 11.5. This sounds like something is going on between where you are uploading your files to and the server for your TDX web apps such that the file reference is not accurate any longer, or maybe your TDX web application server is not able to access the file location for that file any longer after some time.
