Entering Time on T&E Tab
The "Task/Activity" column is blank. How is that information entered when adding time to a ticket?

Time can definitely be added to a ticket task. You can do this either through the Update function of a ticket task, or via Time and Expenses when you choose to enter ticket time. Just select the ticket first via the ticket search button, then you can use the Task/Activity drop down to select a particular task on that ticket to enter time for.
