How to create custom request forms for project requests

To create one or more custom request forms for a project request:

Navigate to: Admin > Portfolio Planning > Request Forms 

  1. Create custom forms to use for project requests as needed. Configure your forms to have all of the desired entry fields, set default values, etc.
  2. Once a form has been created it can be applied to a Service via Admin > Service Catalog > Services 

Keep in mind that custom forms are only used in the Requests application. For the reviewers in Portfolio Planning, the full General tab will be used. Also, any custom attributes created for custom forms will show up when reviewing the request in Portfolio Planning.  More information on how to create custom project attributes can be found here.
